Overview
Job Duties
Provides coordination of care to patients under the direction of the CHFM Medical Directors. Assists with specific in-basket electronic communication, clinical operations, and counsels' patients regarding their health. Virtually asses' patient care and obtains appropriate input as needed to determine by NPP/Physician under the direction of the Clinic Medical Director.
Responsibilities
Essential Functions
โ€ข Researches patient requests for medical advice or prescription refills and data and orders within the scope of the pre-approved practice protocols
โ€ข Identifies opportunities, leads, develops, and coordinates the In-Basket triage centralization process. Ensures timely follow-up and decision making within set guidelines.
โ€ข Collects and assess patient data to determine patient needs and to promote optimal Care. Calls and sends EMR messages to patients regarding coordination of care, health maintenance and appointments after review of clinic provider.
โ€ข Reviews and pends medications, labs, referrals for patients based on Department or practice guidelines
โ€ข Reviews and prioritizes all EPIC in basket tasks and addresses as appropriate
โ€ข Assists with follow up for established patients where continuity of care is required along with timely and appropriate referrals to specialties and ancillary services for patients in need of further medical care and services to be reviewed and signed off by clinic provider
โ€ข Coordinates education and guidance for patients to maximize the achievement of expected outcomes after review by clinic provider.
โ€ข Enters pertinent clinical information in the patient's medical record in a timely and accurate manner and in accordance with established compliance billing guidelines. Documents and closes open encounters, MyChart messages, and other EMR communications in accordance with Institutional requirement and clinic protocols
โ€ข Participates in meetings, functions, events, and committees, as well as in a team of interdisciplinary health care team members
โ€ข Contributes to the development, implementation, and expansion of programs and initiatives, including participation in process improvement, timesaving workflows, and efficiency of in-basket practice.
โ€ข All other duties as assigned

2 years Outpatient Setting preferred
Education Requirements
High School Diploma or GED required
Associates Science or Health Related preferred
MA Certification preferred
Additional: travel up to 50%
