Job description

Patient Navigator

The Patient Navigator serves as a key resource for clients, fostering ongoing engagement in medical care, improving communication, and facilitating access to social services. This role is responsible for guiding patients through the 26Health system with empathy and efficiency, removing barriers to care and supporting timely re-engagement in treatment. Through a patient-centered approach, the Patient Navigator ensures individuals receive the support, education, and resources they need to navigate their healthcare journey successfully.

Duties and responsibilities

  • Performs work from a position that patient care and superior service are the focus and priority in everything we do.
  • Conducts customer service calls to ensure quality patient experience.
  • Schedules patients' appointments and provides information for individual tests.
  • Refers patients to inside services (i.e., medical patients to mental health, or HIV at-risk patients to PrEP, substance abuse counseling) or outside services as dictated by the needs of the patient.
  • Identifies resources for clients to overcome barriers to care, such as transportation, housing, and health insurance coverage.
  • Explains the Marketplace application process to our clients, program requirements and fully informs them of their options.
  • Explains Individual and Small Business Health Packages to uninsured patients and provides them with the opportunity to participate in the program.
  • Ensures all Individual and Small Business Health Package payments are charged in accordance with signed contracts and that all members are scheduled for an initial medical wellness exam.
  • Sends out monthly Customer Satisfaction Surveys; Reviews feedback and identifies areas of opportunity for changes and improvements of patient care, including medical, behavioral, family planning, spa, and any other area of 26Health patient focus.
  • Data Collection
  • Collaborates with Training & Education team to develop and suggest enhancements to training plans that further support patient experience excellence.
  • Performs other duties as required.

Qualifications/Knowledge/Skills/Experience

  • High school diploma or equivalent.
  • Bilingual (English/Spanish) proficiency is required.
  • 2+ years in a related role within a healthcare organization is preferred.
  • Must be willing and able to complete a required 20+ hour online training course and pass final exam for Federal certification prior to assuming Certified Application Counselor duties.
  • Must have the ability to work flexible hours, including evenings and weekends, as needed.
  • Excellent verbal and written communication skills; communication style that is open and fosters trust, credibility and understanding.
  • Proficient in all MS Office applications (Excel, Word, PowerPoint)
  • Experience working with ECW or other EHR system highly preferable.

Working conditions

  • Prolonged periods standing, walking, and/or sitting at a desk and working on a computer may be required. Work may be required in various environments, both inside and outside of the office, as needed by project/activity.

Physical requirements

  • May require the occasional ability to lift up to 25 pounds.
