Registered Nurse — Home Health | URGENT NEED!

Schedule: Monday–Friday, 8:00 AM–5:00 PM + on-call/weekends
On-Call: At least 1 weekend per month
Coverage: Monongalia, Preston, Tucker, Grant & Wetzel Counties, WV
Productivity: 30 points/week
Mileage: $0.54/mile
License: WV RN license


We’re seeking an experienced Home Health Registered Nurse to provide compassionate, high-quality care while supporting patients with chronic, acute, and ongoing healthcare needs in the community.


Key Responsibilities

  • Assess patients, develop and implement individualized care plans, and evaluate outcomes.
  • Provide skilled nursing care focused on health maintenance and disease prevention.
  • Coordinate care with physicians and multidisciplinary healthcare professionals.
  • Manage medications, physician orders, health documentation, and changes in patient condition.
  • Coordinate appointments, hospitalizations, and discharge needs.
  • Supervise LPN/LVN staff and support clinical care activities as needed.
  • Maintain accurate nursing documentation and ensure compliance with health and safety standards.
  • Train new employees and provide annual health and safety training.
  • Ensure unlicensed staff are properly trained on patient-specific health and safety needs.


Requirements

  • Active WV RN license required.
  • Home Health experience required.
  • HCHB experience required.
  • OASIS experience required.
  • Strong clinical assessment, organization, communication, and care-coordination skills.
  • Ability to work independently and collaborate effectively with patients, families, and healthcare teams.
