Qualifications:
  • Associate's Degree or Bachelor's Degree in nursing preferred.
  • Current license as registered professional nurse in New York State.
  • Effective verbal and written communication skills.
  • Strong oral, written and organizational skills
  • Strong computer skills.
  • Committed and effective team worker.
  • Strong commitment to the provision of quality medical care for medically underserved individuals.
  • Proven capacity to work productively in highly collaborative settings and to seek and synthesize input from multiple stakeholders.
  • Knowledge of Public Health practices/procedures, HRSA regulations, federal regulations and EMR.
  • Bi-lingual (English/Spanish) preferred.

Primary duties and responsibilities include but not limited to:
  • Provides nursing care and administers medications and treatments, as ordered by Physician/Nurse Practitioner
  • Works closely with CMO, Director of Operations, Clinical Nurse Manager and Practice Manager to ensure health center operates in coordinated, organized manner such that quality care and services are provided.
  • Serves as a member of Quality Assurance, Infection Control and other committees as deemed necessary.
  • Participates in development, review, revision, implementation and monitoring of policies/procedures at the direction of the Clinical Nurse Manager.
  • Coordinates care and scheduling with Registered Nurse staff at other sites
  • As directed by Clinical Nurse Manager, supervises LPNs, Certified Medical Assistants/clinical support staff in all clinical activities
  • Provides oversight and direction in scheduling and follow-up visits for patients as clinically indicated, including triage.
  • Provides health education to patients and their families as needed.
  • Flexibility to work in both health center locations as assigned.
  • Prepare examination/treatment rooms; measure vital signs (i.e. pulse rate, temperature, blood pressure) as well as height and weight and records information in EMR;
  • Maintain/oversee inventory and order medical supplies and materials.
  • Oversee/maintain log books for quality control.
  • Preload/cues patient charts (med list, problems, allergies, vitals) prior to examination as needed.
  • Clean and sterilize instruments as needed.
  • Operate electronic electrocardiograph (EKG), and other equipment to administer routine diagnostic test or calls a medical facility to schedule patients for tests as needed.
  • Administer treatments and perform routine laboratory tests as needed.
  • Utilize NYSIIS for immunization tracking/upload.
  • Administer injections as requested by MD/NP.
  • Participates in Chronic Care Management/Population Health Management Programs and other programs such as open house/medical screenings and community outreach initiatives as developed by the Center.
  • Triage same day appointments or problem visits.
  • Pre-visit planning.
  • ER/hospital discharge planning, care coordinator and transition of care.
  • Assists Clinical Nurse Manager in recruitment and mentoring of nursing and clinical staff.
  • Utilized Healthix to receive patient hospitalization, ER and rehab tests and procedures and preloads EMR.
  • Completes environmental rounds and equipment inventory.
  • Participates in daily huddle.
  • Schedules bi-annual pharmacy inspection and equipment inspection, as requested by Clinical Nurse Manager.
  • Acts as care manager for patients with complex medical and behavioral health care needs;

  • Compliance with social distancing, adherence to PPE and infection protocols.
