POSITION SUMMARY

The Behavioral Health Care Manager (BHCM) provides high-quality, member-centered behavioral health care management services to Titanium Healthcare's Enhanced Care Management (ECM) population. The BHCM is responsible for engaging members with complex behavioral health and psychosocial needs, coordinating care across medical and behavioral health providers, connecting members to community resources, and promoting improved health outcomes through evidence-based interventions.The BHCM serves as the behavioral health expert on the interdisciplinary care team, providing clinical consultation, supporting care planning, facilitating care transitions, and ensuring members receive comprehensive, integrated care.


WHERE YOU'LL WORK

This position is hybrid. Work from home and go into the field (San Bernardino & Riverside Counties) as needed. Travel is required to conduct in-person member visits within designated areas. You will have full control over your schedule when meeting members. You are eligible for mileage reimbursement for the use of your vehicle for business-related travel. Standard business hours are Monday through Friday from 8:00 am to 5:00 pm.


WHAT YOU'LL DO

Member Care Management

  • Manage ECM members with behavioral health needs, primarily Tier 1 and Tier 2 members
  • Conduct comprehensive biopsychosocial assessments both telephonically and in person
  • Develop therapeutic relationships that promote engagement, self-management, and improved health outcomes
  • Utilize evidence-based interventions including Motivational Interviewing, Behavioral Activation, solution-focused techniques, and brief therapeutic interventions
  • Provide crisis assessment and facilitate warm handoffs to crisis response resources when appropriate
  • Coordinate behavioral health, medical, and social services to ensure integrated, whole-person care
  • Support members during transitions of care, including hospital admissions and discharges
  • Conduct face-to-face visits based on member risk stratification and health plan requirements
  • Maintain ongoing engagement through telephonic outreach, home visits, and community-based encounters

Care Coordination & Clinical Responsibilities

  • Collaborate with primary care providers, specialists, behavioral health providers, hospitals, health plans, and community organizations
  • Participate in Interdisciplinary Care Team (ICT) meetings, Systematic Case Reviews (SCRs), and case conferences
  • Collaborate in the development and ongoing maintenance of member-centered shared care plans
  • Coordinate referrals for behavioral health treatment, substance use treatment, housing, transportation, food insecurity, and other social determinants of health
  • Monitor member progress toward behavioral health and wellness goals
  • Track required assessments, screenings, authorizations, and documentation within the care management platform
  • Ensure timely documentation that meets Titanium Healthcare, MCP, and DHCS requirements

Education & Consultation

  • Educate members and families regarding behavioral health conditions, treatment options, medications, and healthy coping strategies
  • Provide consultation to interdisciplinary team members regarding behavioral health conditions, crisis management, and community resources
  • Assist with staff education regarding behavioral health best practices and evidence-based care approaches
  • Promote integrated behavioral and physical healthcare throughout the organization

Quality & Compliance

  • Maintain compliance with Titanium Healthcare policies, DHCS regulations, ECM requirements, and Managed Care Plan (MCP) standards
  • Support quality improvement initiatives through accurate documentation and timely completion of required member activities
  • Participate in audits, quality reviews, and performance improvement initiatives
  • Complete all assigned documentation within established timelines
  • Perform other duties and projects as assigned

  • Strong knowledge of behavioral health assessment, diagnosis, and treatment planning
  • Understanding of Serious Mental Illness (SMI), Substance Use Disorders (SUD), trauma-informed care, and recovery-oriented practices
  • Knowledge of Enhanced Care Management (ECM), CalAIM, and population health principles
  • Experience utilizing Motivational Interviewing and other evidence-based engagement techniques
  • Excellent crisis assessment and de-escalation skills
  • Strong care coordination and interdisciplinary collaboration skills
  • Ability to navigate complex healthcare and community resource systems
  • Excellent documentation and organizational skills.
  • Ability to prioritize workload and manage multiple competing priorities
  • Proficiency electronic health record systems

    WHAT YOU'LL NEED

    • Master's degree in social work, counseling, psychology, or another related behavioral health field
    • Minimum of 2+ years of behavioral health, care management, case management, or community-based experience
    • Current and valid Driver's License with a clean driving record
    • Reliable transportation and proof of auto insurance

    NICE TO HAVES

    • Experience in Enhanced Care Management (ECM), CalAIM, or managed care
    • Current unrestricted CA LCSW, ACSW, LMFT, AMFT, LPCC, APCC, or equivalent licensure/registration as permitted by program requirements
    • Experience working with individuals experiencing Serious Mental Illness (SMI), Substance Use Disorders (SUD), homelessness, or high-risk populations
    • Experience with eClinicalWorks (eCW)
    • Bilingual in Spanish preferred
