We are seeking a hands-on, relationship-centered Palliative Care Practice Manager to join our growing team and help strengthen access to high-quality palliative care throughout our community.

The Palliative Care Practice Manager supports the operational, administrative, and cultural success of the palliative care program. This role serves as a key operational leader responsible for developing scalable workflows, supporting interdisciplinary collaboration, optimizing patient access and care coordination, and assisting with practice growth initiatives.

What you will do as a Practice Manager:
  • Provide operational oversight and coordination for the palliative care practice
  • Support the development, implementation, and ongoing management of Principal Care Management (PCM) workflows and processes
  • Collaborate with leadership to operationalize new programs, initiatives, and workflow improvements
  • Supervise designated administrative and support staff within the palliative care department
  • Assist with staff onboarding, workflow training, and operational support for staff and providers
  • Serve as a resource for SmartMD and other technology workflow support and training
  • Coordinate intake, referrals, scheduling, and patient flow to promote efficient operations and timely patient access
  • Collaborate with providers, nurse navigators, billing teams, and leadership to support accurate documentation and billing readiness
  • Participate in billing and operational meetings to identify trends, opportunities, and workflow improvements
  • Track PCM enrollment, consent completion, documentation workflows, and monthly operational metrics
  • Identify operational barriers, communication gaps, and workflow inefficiencies and collaborate with the team to develop solutions
  • Promote strong communication and collaboration among interdisciplinary team members
  • Support departmental culture initiatives focused on teamwork, accountability, compassion, and service excellence
  • Assist with process development, standard operating procedures, and workflow standardization
  • Monitor assigned reports, work queues, and operational follow-up items to ensure timely completion
  • Support quality improvement, patient experience, telehealth, and care coordination initiatives
  • Collaborate with leadership on strategic growth and program development opportunities
  • Assist with communication and coordination between external partners, referral sources, and internal departments
What we are looking for in a Practice Manager:
  • Associate’s or Bachelor’s degree in healthcare administration, business, nursing, or a related field preferred
  • Minimum of 3 years of healthcare experience
  • Previous experience in palliative care, hospice, home health, chronic care management, care coordination, medical practice operations, or healthcare leadership preferred
  • Experience with Electronic Health Records (EHR) and healthcare technology systems required
  • Experience with workflow development, process improvement, scheduling coordination, or healthcare operations preferred
  • Strong organizational, communication, and leadership skills
  • Ability to manage multiple priorities in a fast-paced healthcare environment
  • Knowledge of Medicare, healthcare billing workflows, and care coordination models preferred
  • Experience with telehealth workflows, care management programs, and/or PCM/CCM services preferred

Schedule: Flexible hours around a core schedule (Monday-Friday approx. 8:30 AM-5:00 PM)
