As our Transitional Care Coordinator, you’ll serve as a trusted resource for hospitals, physicians, assisted living communities, independent living communities, memory care communities, adult foster homes, skilled nursing facilities, and other healthcare partners throughout the Rogue Valley. 
 
You’ll help patients navigate the transition into Home Health and Hospice while building meaningful partnerships that strengthen our community and expand access to compassionate, life-changing care.

What You’ll Be Doing 
 
Building trusted relationships with hospitals, physician offices, assisted living communities, independent living communities, memory care communities, adult foster homes, skilled nursing facilities, and other community partners 
Educating referral partners about Riverside’s Home Health and Hospice services 
Coordinating smooth patient transitions into care 
Working closely with intake, clinical leadership, providers, and operations to ensure timely admissions 
Following up with referral partners to provide outstanding customer service 
Identifying opportunities to grow referrals through relationship development and community engagement 
Representing Riverside at networking events, community meetings, and educational opportunities 
Serving as a trusted resource for patients, families, providers, and community partners throughout the Rogue Valley

Healthcare experience preferred (Home Health, Hospice, Hospital, Case Management, Senior Living, or related field) 
Experience in business development, community relations, healthcare sales, or referral coordination preferred

Ability to work independently while collaborating with multiple teams 
Professional presentation and customer service skills
