As the Client Services Coordinator for Senior Care and Support Systems at South Coast Business (SCB), you have the unique opportunity to be at the forefront of our mission, providing support and guidance to individuals navigating the complexities of senior care. In this pivotal role, you will be an advocate for our clients, ensuring they receive the resources and care they need to thrive within their communities.
Key Responsibilities

  • Initial Point of Contact: Serve as the welcoming and empathetic first representative for clients via phone, email, or, in-person interactions.
  • Screening and Assessment: Conduct initial interviews and screenings to gather personal, demographic, and historical information (e.g., medical, social, legal) to determine needs and eligibility for various programs.
  • Information and Referral: Provide clients with brief education, information, and/referrals to appropriate internal programs or external community services, embodying the "no wrong door" philosophy of seamless access.
  • Documentation and Data Entry: Process all intake forms, ensuring accurate and. confidential record-keeping in client management systems (CRM/EHR) in compliance with regulations like HIPAA.
  • Navigation and Coordination: Work closely with a multidisciplinary team and external agencies to coordinate services and ensure a smooth transition for the client, often acting as a liaison between the client and service providers.
  • Crisis Handling: Handle crisis calls or difficult client interactions with patience, empathy, and critical thinking skills, developing immediate safety plans if necessary.
  • OutreachandAdvocacy:Participate 'in community outreach efforts to promote awareness of available services and build relationships with outside agencies to facilitate referrals.
  • Required Qualifications
    • High school diploma required, college diploma or bachelor's degree in social work, psychology, or a related human services field preferred.
    • Experience in client services or social work, preferably in a senior care or healthcare setting.
    • Exceptional verbal and written communication skills, with a patient and compassionate approach.
    • Strong interpersonal skills with the ability to build rapport with diverse populations.
    • Excellent organizational skills and attention to detail for managing multiple tasks and confidential information.
    • Proficient in standard office software, including Microsoft Office Suite, and be comfortable with data entry systems.
