Description:

The CCBHC Service Navigator helps individuals who are seeking services with MHR by reviewing intake screeners and making referrals to community resources and service programs as indicated by their screeners. 


The CCBHC Service Navigator is expected to have a general understanding of all programs offered by the CCBHC so that clients can be educated about the programs and connected effectively to care that best fits their recovery goals. 

Requirements:

Full Time, 40 hours, non-exempt

Starting pay: $17.37/hr

Minimum of a high school diploma

Valid driver's license

Possess or be willing to obtain certification in Motivational Interviewing and SBIRT (Screening, Brief Intervention and Referral to Treatment

Management reserves the right to add, modify, change, or rescind the work assignments of different positions and to make reasonable accommodations so that qualified employees can perform the essential functions of the job. 


1. Meet with clients upon intake to review all intake screeners, address SDOH needs and link the client to an appropriate community resource to reduce SDOH factors

2. Will have a working knowledge of all CCBHC services and will refer the individual to programs within the CCBHC that can support immediate needs and reduce barriers to care. 

3. Will conduct brief intervention with clients regarding problematic alcohol use if indicated in AUDIT-C screener. 

4. Will discuss programs or groups offered at the agency or within the community to support recovery needs of the client

5. Will complete the Health Screening with the client and will obtain basic health information, such as Blood Pressure and weight, and will help to navigate a referral to a primary care physician if needed. 

6. Will assess a client's desire to create an advance directive and will link the client to the agency resource for completion. 

7. May be asked to complete 24-hour follow-up calls for clients exiting acute care or ED care to ensure needs are met. 

8. Will utilize the EHR for documentation of interactions, referrals made and follow ups with clients
