Job description

Nurse Practitioner

Are you an experienced Nurse Practitioner in [location] area looking for a new opportunity? Are you interested in working for a company that offers flex schedules and fixed payment per visit in the health plan members' homes? If you answered "YES" to any of these questions – this is the position for you!

Daily Responsibilities:

  • Position requires face-to-face interaction with insurance members in their homes to conduct a health risk assessment which includes a brief physical and will be completed on laptop/tablet that is provided. The visits normally take about an hour and can be scheduled any day of the week. There is no prescribing or treatment plans involved.
  • Responsible for submitting the completed assessment within 24 hours of the visit and working with our Provider Management team to ensure excellent quality results.
  • Flexible scheduling -- you will be responsible for maintaining and managing the days and hours that you work using our online scheduling system.

Qualifications:

  • Must be Nationally Board Certified
  • Must hold an Active Nurse Practitioners license in the desired state (new grads accepted but only those that have obtained their license can start the process)
  • Must be willing to give 10 hours or more of availability per week
  • Must be willing to cover an entire county and ideally have the flexibility to travel to neighboring counties as needed (unless it is a very populated area where the membership is concentrated)
  • Must be able to conduct assessments using electronic technology (iPad or laptop)
  • Must have a specialty in any of the following: Family, Adult, Gerontology, or Acute Care (no Psychiatric, Pediatric, Women's Health or Neonatal accepted)
  • Must be willing to provide Immunizations records/documentation
  • CPR/BLS for Healthcare Provider
