We are seeking a Full-Time Unit Ward Clerk / Health Information Management (HIM) team member who is organized, dependable, detail-oriented, and committed to providing the highest level of service to our guests and nursing team.


This position is ideal for a Certified Nursing Assistant (CNA) who enjoys working with residents and families while also having strong organizational, computer, and clerical skills.

Requirements

The Unit Ward Clerk / Health Information Management (HIM) will:

  • Organize and maintain the clerical functions of the nursing units
  • Maintain resident medical records and ensure documentation is properly filed and organized
  • Review medical records for completeness and assist with obtaining missing documentation and required signatures
  • Assemble charts and documents for new admissions, transfers, and discharges
  • Handle medical records requests according to facility procedures and HIPAA requirements
  • Maintain confidentiality of all resident health information
  • Answer telephone calls and promptly communicate messages and notices to the appropriate team members
  • Schedule resident follow-up appointments and outside services
  • Coordinate resident transportation for appointments
  • Schedule dental, podiatry, vision, and other resident services as required
  • Assist with coordinating lab, x-ray, therapy, and other ordered services
  • Take pictures of new residents and update resident information as needed
  • Maintain nursing stations, forms, files, and required notices in an organized manner
  • Assist residents, families, nursing staff, physicians, and other departments as needed
  • Uphold Resident Rights at all times
  • Perform other duties as assigned

Unit Ward Clerk / Health Information Management (HIM) must have the following:

  • Certified Nursing Assistant (CNA) Certification per state requirements
  • High school diploma or equivalent
  • Strong organizational skills and attention to detail
  • Computer, email, and data-entry skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Excellent communication and customer service skills
  • Ability to maintain confidentiality of resident information
  • Experience working in a long-term care, skilled nursing, rehabilitation, or hospital setting preferred
  • Previous medical records, HIM, or Unit/Ward Clerk experience preferred
  • Compliant with the state's Health Care Worker Background Check requirements with respect to criminal history
