Summary Statement

This Health/Human Service Case Manager I position is in DMMA's Central Intake Unit (CIU). The incumbent will be responsible for processing detailed referrals, eliciting confidential and highly sensitive financial and medical information from applicants and applicants' representatives, data-entering pertinent information, facilitating the assignment of applications to financial and medical eligibility staff for all Long Term Care (LTC) Medicaid programs, determining eligibility for Medicaid and Supplemental Nutrition Assistance Program (SNAP) benefits, and maintaining the CIU resource mailbox.\r\n\r\n

Essential Functions

\r\n\r\nEssential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class. Since class specifications are descriptive and not restrictive, employees can complete job duties of a similar kind not specifically listed here.\r\n\r\n\r\nThis class is distinguished from a licensed Social Worker as employees do not perform the following duties: diagnose individuals for mental, emotional, behavioral, addictive, and/or developmental disorders and disabilities; perform related client assessment or evaluation; provide psychotherapeutic treatment; or develop and implement assessment-based treatment plans. An employee in this class may participate as a member of a multi-disciplinary team to implement behavioral health services or treatment plans in accordance with Delaware Code, Title 24.\r\n\r\n\r\n\r\n\r\n\r\n Interviews clients, family members, and others to determine eligibility and connect clients with health and human services.\r\n \r\n Develops a service plan to aid and assist a client in receiving care and assistance needed in collaboration with hospitals, clinics, agencies, and other appropriate resources (Service plans may include job training and employability, housing, general public assistance, in-home services and supports or home-delivered meals, de-escalation techniques, peer services, or skill development).\r\n \r\n Evaluates information derived from client interviews, case history, and other pertinent sources and refers to proper channels for review.\r\n \r\n Provides guidance and relevant information to clients and families prior to receiving requested service and assistance.\r\n \r\n Coordinates with other agencies, hospitals, clinics, and resources to expedite the process on behalf of the client.\r\n \r\n Prepares case histories and reports and maintains records and chronological logs of case activities.\r\n \r\n Uses automated information systems and computer software to research, review, evaluate, and report on health/human service data.\r\n \r\n Communicates and interacts effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.\r\n \r\n Performs other related duties as required.\r\n\r\n\r\n\r\n\r\n\r\n\r\n\r\n \r\n \r\n\r\n\r\n\r\n

Job Requirements

JOB REQUIREMENTS for Health/Human Service Case Manager I\r\nApplicants must have education, training and/or experience demonstrating competence in each of the following areas:\r\n\r\n Possession of an Associate's degree or higher in Behavioral or Social Science or related field.\r\n\r\n OR\r\n\r\n Three years' experience in health or human services supportsuch asinterviewing clients and assessing personal, health, social or financial needs in accordance with program requirementsor coordinating with community resources to obtain client services.\r\n Six months' experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.\r\n Six months' experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.\r\n Six months' experience in narrative report writing.\r\n\r\n

This position is a classification organized under an exclusive bargaining representative (labor organization) that has been elected by employees as their representative for collective bargaining and other applicableterms and conditions of employment, in accordance with Title 29, Chapter 59 and Title 19, Chapter 13 and 16. This position is covered by a collective bargaining agreement based on its individual terms.\r\n\r\nAll new hires are required to report to the Statewide New Employee Orientation (SNEO) on their first day of employment. Sessions are held at designated locations in Kent or New Castle County.\r\nApplicants must be legally authorized to work in the United States.
