FTE Status: 0.60 FTE (24 hours/week)
Work Schedule: Hours flexible between 7:30a-4:00p, Monday -Friday, aligned with school needs

Job Description:
The Community Health Worker will promote health equity within the identified patient population. They will focus to discover and recognize social determinants of health disparities through patient engagement to achieve population health initiatives. Will work to identify community level barriers to cultivate additional partnerships to bridge gaps for the targeted population. Goal is to advocate for patient needs, perform outreach, education, and identify opportunities to enhance care coordination. Other responsibilities including maintaining accurate health data on identified population, develop outcome measures, and make connections to community and health care resources.
Qualifications:
Education:
High School Diploma or equivalent required.
Certification/Registration/Licensure:
C3 training within one year of hire.
Experience:
Minimum one year of experience in health care or human service-related field. Familiar with community resources and speaks the identified population language helpful but not required.
