The Health Coach is responsible for providing a safe environment for patients to explore their health and lifestyle choices. The Coach will use motivational interviewing skills to review patients' goals and will collaboratively develop healthcare meal, exercise or wellness plans tailored to each patient's needs. The Coach will provide recommendations to reduce or eliminate unhealthy or high-risk behaviors.
