RN Health Coach

RN Health Coach works under the supervision of the department's physicians. Serves in an expanded nursing role to collaborate with patients and their Primary Care Providers to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status. Integrates evidence-based clinical guidelines, preventive guidelines, and protocols in the development of treatment plans. Develops systems of care that monitor patient progress and promote early intervention in acute care situations. Works effectively with other members of the health care team to optimize interventions.

Responsibilities

Health Coach position is responsible for maintaining the TCM follow up calls and visits. They also do patient education for Diabetes and Hypertension. They help patients with financial assistance for medications, which we have many. They arrange Home Health Care for patients when ordered by physicians. Set up orders for patients that are due for mammograms, colonoscopies and diabetic management.

Demonstrate ability to:

  • Function effectively in a fluid, dynamic, and rapidly changing environment.

Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving and decision-making, priority setting, work delegation and work organization.

Qualifications

Minimum Education and Experience Requirements: Licensed as RN in Florida. ADN required; BSN preferred. Bachelor's degree in health care preferred. Case Manager Certification preferred. Three to five years' experience in clinical nursing, and case management. Knowledge of the basic concepts and principles of managed care required.

Critical thinking skills and ability to analyze complex data sets required. General computer knowledge and capability to use computers required. Demonstrates customer focused interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary health care team, community agencies, patients, and families with diverse opinions, values, and religious and cultural ideals.

Motor Vehicle Operator Designation: Employees in this position: Will not operate vehicles for an assigned business purpose NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job. Please indicate the appropriate operator designation on the Request for Personnel (RFP) form at the time a RFP is submitted to post the position.

Licensure/Certification/Registration: Current licensure by the State of Florida as a Registered Nurse. Basic Life support certification or obtained at first opportunity with employment. OCN strongly preferred.
