Full Time - Exempt

AndHealth is on a mission to radically improve access and outcomes for the most challenging chronic health conditions, with the goal of making world-class specialty care accessible and affordable to all. We partner with health systems, community health centers and independent specialists to remove barriers to care to ensure all people have access to the care they deserve.
We are looking for a health coach who can play a key role in helping our patients reverse their chronic disease. In this role, you will utilize your specialized education in diet and nutrition to help guide and coach patients through aspects of their journey with AndHealth to achieve health outcomes. You will work with our Physicians, Nurse Practitioners, and Patient Success in providing a personal care plan for the patients. Ideally looking for someone with a Registered dietitian or Certified Nutrition Specialist background.
What you’ll do in the role:
  • Utilize evidenced-based health coaching interventions to improve patient engagement and outcomes.
  • Guide patient through the healthcare provider created treatment plan, and partner with providers to provide safe, effective coaching and care to patients.
  • Collaborate with the Care Team to develop integrated treatment plans, and partner with medical providers to provide safe, effective treatment and coaching to patients.
  • Provide AndHealth integrated health coaching treatment approaches to a full patient panel, including strategies to target sleep, stress, nutrition, movement, and medication adherence. This is done under the guidance of care plans designed by medical practitioners.
  • Provide behavior change coaching and care to a full patient panel, using stages of change. 
  • Collect, interpret, and track patient-reported outcomes and wearable data, and use this data to inform individualized treatment plans, in collaboration with the patient’s medical provider.
  • Support patients with personalized, medically necessary, and informed nutrition plans under the direction of a registered dietician to achieve less severity and frequency of migraine attacks and/or autoimmune flares.
  • Conduct virtual, synchronous treatment sessions with patients, to evaluate progress and adapt interventions to the patient’s needs.
  • Engage patients in asynchronous and individualized coaching as needed using technology platforms.
  • Follow clinical workflows and processes using an EHR and other key tools to effectively document patient outreach and progress.
  • Actively contribute to process improvements by providing regular continuous feedback to internal stakeholders.
  • Assist with special projects as needed.
Education & Licensure Requirements:
  • Bachelor’s degree is preferred.
  • Completion of a health coach training program that qualifies you to sit for the NBHWC Certification exam preferred.
Other Skills or Qualifications:
  • Established experience working with patients with chronic diseases.
  • Demonstrated experience working within a medical model and/or an interdisciplinary team.
  • Demonstrated experience and comfortability working with patient data, including utilizing, and adapting to various technology platforms.
