Job description

  • Work under the direct supervision of a pharmacist to support patients throughout various phases of care.

  • Collect the best possible medication history to support accurate and timely medication reconciliation by the care team.

  • Facilitate the provision of discharge medications, helping ensure patients have access to necessary medications upon leaving the hospital.

  • Collaborate closely with pharmacists, nurses, physicians, and other care team members to ensure safe and effective medication use during transitions of care.

  • Support efforts to reduce medication-related readmissions and improve continuity of care.


Education:

  • High school diploma or equivalent required. 
  • Graduate of a pharmacy technician program preferred.

Experience:  

  • Minimum 2 years of experience as a pharmacy technician required; retail hospital experience preferred.
  • Experience applying customer service behaviors and communication skills required.
  • Basic familiarity with computers and keyboard skills and ability to navigate electronic systems applicable to job functions.

  • Current applicable state Pharmacy Technician Licensure or Pharmacy Intern License required prior to date of hire and maintained thereafter.

USD $29.67 - USD $38.77 /Hr.
