Position Overview

Join Heal Motion Care as a dedicated Full-Time Outpatient Physical Therapist, where you will be instrumental in enhancing patients' recovery journeys. Key Responsibilities
  • Conduct detailed Physical Therapy evaluations for children and adults, assessing their individual needs and capabilities.
  • Craft individualized treatment plans that align with patient goals and promote recovery, utilizing evidence-based practices.
  • Regularly monitor and reassess patient progress through standard clinical measures to determine treatment effectiveness and make necessary adjustments.
  • Complete documentation in a timely manner using our electronic health record system, including SOAP notes and progress summaries.
  • Educate patients and their families about the recovery process, setting realistic expectations and empowering them to actively participate in rehabilitation.
  • Collaborate closely with multidisciplinary teams to ensure clients receiving multiple services benefit from a cohesive treatment approach.
  • Uphold the company’s values by fostering a supportive and positive patient experience, acting as an ambassador for Heal Motion Care.
Patient Population / Client Focus

Qualifications

  • Ability to deliver evaluations and treatments in both in-person and telehealth settings as clinically appropriate.

Compensation and Benefits
