Team 1: Bayville, Bethpage, Brookville, East Norwich, Glen Cove, Glen Head, Glenwood Landing, Greenvale, Hicksville, Jericho, Lattingtown, Laurel Hollow , Locus Valley, Massapequa, Mill Neck, Muttontown, Oyster Bay, Plainedge, Plainview, Seacliff, Syosset,  & Woodbury.

  • 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details

Our Home Care RN's:

  • Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
  • Measure vital signs, Administer IVs, change dressings, clean wounds as needed
  • Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
  • Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
  • Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.
  • Provide care to the most upmost standard

Minimum Qualifications:

  • New York State RN license
  • Minimum of one year of recent hands on experience as a Registered Nurse.
  • Basic Life Support Certification
  • Valid New York State Driver's License and reliable transportation
