This role is primarily onsite, delivering therapy in clients' homes throughout the vibrant city of San Francisco. You will engage in hands-on therapy while also assessing the home environment to ensure it is conducive to rehabilitation and independence.

As a Physical Therapist at Head Hunters Chiropractic Clinic, you will:

• Direct and implement individualized physical therapy treatment plans that prevent disability, relieve pain, and restore muscle function, strictly following the prescribed plan of care from physicians.
• Oversee and guide Physical Therapy Assistants in executing therapy modalities and ensure compliance with the treatment plans.
• Conduct comprehensive evaluations to assess functional abilities through tests related to muscle, nerve, and joint functions, and adjust rehabilitation strategies accordingly.
• Perform in-depth evaluations of the home environment for rehabilitation needs and recommend modifications to foster client autonomy.
• Deliver therapeutic exercises, both manual and non-manual, and provide education to clients and families regarding exercise regimens and equipment usage.
• Administer specialized treatments such as traction and massage, while utilizing physical agents to enhance rehabilitation.
• Record observations, evaluate clients’ progress through clinical documentation, and actively participate in interdisciplinary team case conferences for comprehensive patient care development.
• Instruct clients and their families in managing the use of physical therapy devices and ensure clear communication regarding the care process.
• Participate in the continuous review and updating of individual care plans, utilizing analytical skills to modify approaches as necessary.

Your services will primarily support clients recovering from surgeries, those managing chronic conditions, or those experiencing mobility challenges due to age or health issues. The emphasis is on delivering compassionate care that encourages independence and enhances the quality of life for each patient. By providing tailored therapy in a familiar environment, you will enable our clients to achieve their rehabilitation goals while fostering a supportive relationship with their families.

To thrive in this role, you should possess:

• A Doctorate in Physical Therapy from an accredited institution.
• Current and valid Physical Therapist licensure in the state of California.
• A minimum of one year of practical experience as a Physical Therapist in an acute care or home care environment.
• Current CPR certification and a valid driver’s license with automobile insurance.
• Excellent communication skills that enhance patient engagement and teamwork.
• A strong commitment to clinical excellence, demonstrating integrity and advocacy for clients.

Candidates with experience in a home care setting or expertise in geriatric therapy will be preferred. Familiarity with federal, state, and governing agency regulations is advantageous, as is a history of collaborative practice within an interdisciplinary team.
