A rapidly growing healthcare organization is seeking a Nurse Practitioner to provide comprehensive, relationship-based care for Medicare patients.
This is an opportunity to practice medicine the way many clinicians envision it-focusing on quality instead of volume. You'll build meaningful relationships with patients, spend the time necessary to address complex medical needs, and collaborate with an interdisciplinary care team dedicated to improving outcomes while reducing avoidable hospitalizations.

What You'll Do
You'll manage a panel of medically complex Medicare patients by delivering comprehensive, patient-centered care across clinic, dialysis center, and home settings.
Responsibilities include:
  • Provide longitudinal primary care for patients with CKD and ESKD
  • Perform comprehensive health assessments and annual health risk assessments
  • Develop individualized care plans that support improved health outcomes
  • Educate patients and families about diagnoses, medications, and disease management
  • Coordinate care with nephrologists, primary care providers, specialists, caregivers, and community resources
  • Manage chronic conditions, medication regimens, and post-discharge follow-up
  • Help prevent unnecessary emergency department visits and hospital readmissions
  • Accurately document clinical encounters and HCC coding
  • Order and interpret diagnostic tests as appropriate
  • Practice independently while collaborating closely with physicians and an integrated care team
  • Maintain compliance with HIPAA and all regulatory requirements
Practice Environment
This position offers a blend of:
  • Dialysis center visits
  • Nephrology clinic visits
  • In-home patient visits
  • Occasional telehealth appointments

Travel is primarily within your assigned territory, with occasional coverage in nearby regions based on business needs.
Requirements
What We're Looking For
Required
  • Active California Nurse Practitioner license
  • Board Certification (AANP or ANCC)
  • Current DEA (or ability to obtain)
  • Current BLS/CPR certification
  • Valid California driver's license
  • Minimum two years of Nurse Practitioner experience in one or more of the following:
    • Primary Care
    • Family Medicine
    • Internal Medicine
    • Nephrology
    • Geriatrics
    • Palliative Care
  • Experience managing patients with chronic diseases
  • Working knowledge of HCC coding and value-based care documentation
Preferred
  • Home-based primary care experience
  • Experience caring for Medicare populations
  • Value-based care experience
  • CKD/ESKD patient management
  • Care coordination experience
