• Conduct comprehensive assessments to identify the immediate and ongoing needs of our clients, ensuring personalized treatment plans are developed.
• Facilitate effective communication among various stakeholders, including supervisors, other healthcare providers, family members, and guardians, to monitor and enhance client progress.
• Develop and provide a tentative schedule to supervisors that outlines appointments, home visits, and observations aimed at monitoring the efficacy of treatment.
• Coordinate and perform necessary home visits, as well as chart reviews and data analysis, to collect relevant information key to our clients' progress.
• Ensure the effective implementation of individual program plans, making necessary adjustments based on client feedback and observed outcomes.
• Participate actively in Interdisciplinary Team Meetings and contribute to discussions regarding best practices and client care strategies.
• Complete timely and accurate monthly and quarterly progress notes, documenting client advancements and overall treatment developments.
• Organize and conduct training for staff members, ensuring consistency and competence in the implementation of treatment programs and documentation standards.
• Provide continued support through follow-up interviews and observations, ensuring that client needs are met while fostering an environment of trust and rapport.
• Maintain precise and accessible records for all clients, guaranteeing that vital information is available when needed.
• Act as a liaison with the broader mental health community, collaborating to support ongoing development and resource allocation for goals.
• Assist in the creation of effective training materials aimed at improving our programs and services for individuals.

You will be working with a diverse population that includes both adults and youth facing a variety of mental health challenges. Our patient demographics require counselors who have a strong understanding of different therapeutic modalities, particularly in areas such as family therapy, couples counseling, trauma recovery, and substance misuse treatment. This position allows you to make a meaningful impact on the mental health and well-being of individuals across various life stages and circumstances.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline is a prerequisite for this role.
• Applicants must have active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
• At least two years of relevant counseling experience, preferably in a clinical setting, is essential for this position.
• Candidates with specialization in areas such as family therapy or trauma are highly preferred, enhancing our ability to serve diverse client needs.
• A thorough understanding of legal and ethical standards within the mental health field is required.
• Excellent interpersonal skills and a proven ability to establish rapport with clients from various backgrounds are crucial.

• Familiarity with electronic medical records (EMR) systems and related technologies is preferred and will assist in streamlining client management.

• Experience in telehealth counseling is highly desirable due to the remote nature of this position.
