St. Ann’s Community is currently hiring a full time Program Assistant (CNA) in the Adult Day Services Home located on the Irondequoit Campus. The purpose of this position is to perform routine procedures according to established policies and care plans.

Job Description:

  • Responsible for bathing, grooming and performing toileting rounds every 2 hours 
  • Provides assistance with transfer, ambulation, positioning and lifting
  • Assists participants with meals and supervises, encourages participants who  requires cueing with meals, completing meal nourishment documentation in EHR, including Family-style Dining Program
  • Assist with recreational activities and programs throughout each program day, including transports to chapel and field trips or community integrated events
  • Responsible to attend required annual in-service programs; (6) hours of in-house in-service every (6) months.
  • Assists with admissions, transfers and discharge of participants
  • Reports any change in participant's condition or behavior in a timely manner

Candidate Requirements: 

  • Current/Valid NYS Nursing Assistant Certification required
  • High School Diploma or GED required
  • 6 Months of related experience
