Compensation Package

Position Summary

As a Registered Nurse (RN) with GrandCare, you'll play a vital role in coordinating and delivering high-quality home health services. Using your clinical expertise and community health knowledge, you'll help patients and families navigate recovery while ensuring care is delivered safely, effectively, and compassionately. This role offers the unique opportunity to work closely with patients in their home environment, develop individualized care plans, and support positive outcomes through every stage of recovery.

Key Responsibilities
  • Develop and initiate the plan of care and assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver(s)
  • Initiate proper preventative and rehabilitative nursing procedures and provide services that are ordered by the physician as indicated in the plan of care
  • Provide patient and caregiver education
  • Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient
  • Ensure that arrangements for equipment and other necessary items and services are available
  • Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aides (HHAs)
Qualifications
  • Valid California Registered Nurse License
  • 2+ years of nursing experience (home health experience preferred)
  • Current CPR certification
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
