Compensation Package: Competitive piece-rate pay with multiple bonus opportunities:

  • Per Visit: $70 - $130 per completed visit (good-faith estimate; varies by visit type)

  • Drive Time: $25 per hour

  • Mileage Reimbursement: $0.70 per mile

  • Productivity Bonus: Earn additional incentives based on performance

  • Documentation Bonus: Reward for timely submission of documentation

Job Summary

As a Physical Therapist (PT) with GrandCare, you will provide in-home physical therapy services according to a physician's plan of care. You'll help patients regain mobility and independence after surgery, making a real difference in their recovery.

Responsibilities

  • Perform home health physical therapy evaluations and develop care plans

  • Provide therapeutic treatment and instruct patients/caregivers

  • Monitor progress and report changes to physicians

  • Participate in discharge planning and interdisciplinary case conferences

  • Supervise Physical Therapy Assistants per state regulations

Requirements

  • Current California Physical Therapy License

  • 2+ years of physical therapy experience (home health preferred)

  • CPR certification

  • Valid driver's license and insured vehicle
