This role is a remote position, allowing for a flexible work environment that contributes to a healthy work-life balance. Our clinical team members utilize advanced telehealth platforms to conduct therapy sessions, ensuring that each client receives personalized attention in the comfort of their own space. The hospice center setting necessitates a unique approach to therapy, focusing on end-of-life care and the emotional support of patients and their families.

As a Licensed Independent Clinical Social Worker, your daily activities will include:

• Collaborating closely with physicians and interdisciplinary teams to monitor and report on patient progress, needs, and any emerging concerns or complications.
• Maintaining detailed and timely clinical documentation, including progress notes that adhere to department standards and facilitate effective communication across care teams.
• Providing immediate emotional support and crisis intervention for clients experiencing acute distress or significant changes in their mental health status.
• Regularly evaluating client progress in relation to established treatment goals, making necessary adjustments to care plans to optimize therapeutic outcomes.
• Coordinating treatment approaches with clients' families and support systems to ensure comprehensive discharge planning and ongoing support.

This position primarily serves clients in a hospice care setting, often dealing with complex emotional and psychological needs as they face end-of-life issues. You will have the opportunity to work with diverse populations, including individuals contending with terminal illnesses, their families, and caregivers. The essence of your work will be facilitating coping strategies and enhancing the quality of life during challenging times.

To be considered for this role, candidates must possess the following qualifications:

• A Master's degree in Social Work, Counseling, Psychology, or a related field.
• An active state license as a Licensed Independent Clinical Social Worker (LICSW) or an equivalent credential.
• A minimum of 3 years of professional experience in an outpatient or clinical setting, ideally with a focus on hospice care or end-of-life issues.
• The ability to work autonomously and exhibit strong organizational skills, while also collaborating effectively within a multidisciplinary team.
• Proficient computer skills, including expertise in word processing, electronic medical records, and telehealth technology.

Although the minimum experience is three years, candidates with additional experience in grief counseling, crisis intervention, or palliative care will be given preference. Familiarity with electronic health records (EHR) systems and teletherapy tools is also advantageous.

The annual salary range for this full-time position is between $100,000 and $105,000, commensurate with experience. In addition to competitive pay, we offer a rich benefits package that includes:
