The Opportunity
ย 
Position Summary:
Responsible for assisting clients and families with social and financial issues related to continuing care admissions or return to the community. Collaborating with the Community Care Coordinator, Transition Services to assist withย the development of the discharge plan. Communicating with the client and family about progress toward the appropriate discharge location.

Program Summary:
Neuro Rehabilitation Program - for adults who have had a neurological injury, such as a stroke, and require additional assessment, recuperation and rehabilitation before returning to the community.

Qualifications & Experience:
- Graduation from a recognized degree program in Social Work or equivalent program.
- Registered with Alberta Association of Social Workers.
- Driver's License
- Minimum of 2 years experience in a combination of clinical and continuing care roles.
- Gerontology experience is preferred.
