Description

INTAKE SPECIALIST FOR RAPID SERVICE RESPONSE PROGRAM IN SOUTHWEST PHILADELPHIA

The Intake Specialist is responsible for receiving and coordinating the referral and assignment and first visit process for all referrals to the Rapid Service Response Program (RSR) at Gemma Services, a brief home-visiting intervention. Oversees all referral and intake operations for RSR; ensures referral processing, initial contact scheduling, and documentation meet DHS and RSR standards while modeling Gemma's UNITED values and the Gemma Way. Works closely with staff to maintain compliance, chart audits, workflow recommendations, and thorough knowledge of regulations and internal policies. Coordinates with Case Workers, Family Advocate Partners and Supervisors frequently to ensure families have their first meeting within 24 hours of referral receipt.

The Intake Specialist will:

  • Ensure initial home meeting occurs within one business day; coordinate attendance/invitation of the DHS Intake Worker, assigned RSR Case Worker and one of the Family Advocate Partner.
  • Coordinate with Supervisors around locations of all cases, caseloads at minimum twice daily.
  • Monitor timeliness and data quality; maintain real-time PFDS entries and internal dashboards
  • Receive, process and document referrals from DHS Prevention Referral Unit
  • Collaborate with Program Supervisors, Family Advocate Partner and QA to review trends and implement improvements; participate in audits/site visits and support Plans of Improvement (POIs).

This is a hybrid role: Monday โ€“ Friday, full-time position that is based out of our Preheim Center in Southwest Philadelphia. Flexible schedule preferred.

Experience and Education: Must have a minimum of three (3) years' experience working with intake/case management in a child welfare setting. A bachelor's degree in social service, population health or related field is required; master's degree preferred.
