Job Description:

We are seeking a passionate and skilled Provider to join our collaborative team at our Hutchinson location. The ideal candidate will embrace the principles of Functional Medicine, value patient education and empowerment, and thrive in a team-based care environment. This position offers the unique opportunity to spend meaningful time with patients (30-60 minute appointments), practice medicine without insurance constraints, and focus on sustainable, root-cause solutions rather than symptom management alone.


Responsibilities:

  • Create individualized plans of care for patients that focus on root cause analysis using our 5 Pillars of Health framework (Sleep, Movement, Nutrition, Stress, Detoxification)
  • Educate and empower patients to achieve positive and lasting transformational change through collaborative partnerships rather than a traditional doctor-patient hierarchy
  • Collaborate with other Providers on the team to benefit patients with unique/challenging cases, participating in case review discussions and peer learning opportunities
  • Conduct comprehensive patient assessments including detailed health histories, lifestyle evaluations, and appropriate diagnostic testing (specialty labs, DEXA scans, etc.)
  • Utilize PHW's extensive toolkit of interventions including supplements, IV therapies, lifestyle modifications, and when necessary, prescription medications
  • Work closely with our Medical Team (RNs, MAs) and Care Coordinator Team to ensure seamless patient experiences and continuity of care
  • Refer patients to Movement Center services (Chiropractic, Massage Therapy, MNRI, Personal Training) when appropriate to support their wellness journey
  • Participate in patient education initiatives including wellness workshops, podcast recordings, and community events
  • Maintain accurate and thorough documentation in our EMR system (Cerbo) that reflects the patient's complete care journey
  • Embody and promote PHW's +IMPACT Framework in all interactions with patients and team members
  • Support the professional development and mentorship of other team members
  • Contribute to the ongoing refinement of clinical protocols and best practices within the functional medicine model


Required Skills and Competencies:

  • Maintain impeccable written and verbal communication with teammates and patients
  • Active and unrestricted license as an APRN in the state of Kansas
  • Strong foundation in functional medicine principles or demonstrated commitment to learning this approach (certification from IFM, or similar organizations preferred)
  • Minimum of 2 years clinical experience preferred; new graduates with strong functional medicine training will be considered
  • Ability to think critically and creatively about complex, multi-system health challenges
  • Genuine passion for patient education and the belief that patients are capable of transforming their own health
  • Strong organizational skills and ability to manage time effectively across patient care, documentation, and team collaboration
  • Ability to quickly get up-to-speed on internal software (Cerbo EMR, Bamboo HR, Google Workspace)
  • Willingness to practice medicine that prioritizes natural interventions and lifestyle modifications while maintaining appropriate use of conventional treatments when indicated
