Role Summary

Our client, a home health care provider serving a diverse Southern California patient population, is seeking a Physical Therapist to join their clinical team. This full-time role focuses on delivering high-quality, patient-centered physical therapy services in patients’ homes.

The Physical Therapist will be responsible for assessing patient needs, developing individualized treatment plans, and delivering rehabilitation services that support recovery, mobility improvement, and long-term functional independence. This role emphasizes autonomy, flexibility, and strong interdisciplinary collaboration within a home health care setting.


Key Responsibilities
  • Conduct comprehensive patient evaluations in home settings to assess functional needs and mobility limitations
  • Develop and implement individualized physical therapy treatment plans
  • Deliver hands-on therapeutic interventions to improve recovery, mobility, and quality of life
  • Monitor patient progress and adjust care plans as needed
  • Collaborate with a multidisciplinary care team to ensure coordinated patient care
  • Educate patients and families on therapeutic exercises and home-based care strategies
  • Maintain accurate clinical documentation and adhere to billing and compliance standards
  • Support patient-centered care delivery focused on independence and long-term wellness

Required Qualifications
  • Graduation from an accredited Physical Therapy program (CAPTE-approved or equivalent)
  • Active Physical Therapist license in the state of practice
  • Minimum of approximately 9 months of clinical experience preferred (new graduates considered)
  • Valid CPR certification (required or must be maintained)
  • Ability to travel to patient homes as part of job responsibilities
  • Valid driver’s license and reliable transportation
  • Strong clinical competency and commitment to high-quality patient care
