Job description

Job Title: Supportive Services Coordinator - Transportation

Department: Supportive Services

Status: Full-Time

FLSA: Non-Exempt


Job Summary:

The Supportive Services Coordinator assures access to Transportation and Educational Services so that transportation is not a barrier to healthcare and supportive services. Transportation is provided through funding sources such as Broadway Cares, Ryan White B (RWB) and other resources, under a cascading model to ensure RWB funds are the payor of last resort.

This service supports transportation to medical, dental, behavioral health, and other healthโ€‘related services that promote positive health outcomes. The Supportive Services Coordinator is also responsible for providing educational services that help people maintain as much independence as possible. The Medical Transportation Coordinator is responsible for coordinating and organizing transportation resources and developing/implementing educational opportunities for clients.


Duties/Responsibilities:

Eligibility Determinations

  • Assess transportation needs, determine what mode of eligible transportation best meets the personโ€™s needs, and develop plans that address educational needs and secure transportation.
  • In compliance with the AIDS Institute Supportive Services standards, contribute in ongoing evaluation led by Medical Care manager.
  • Educate participants and their stakeholders to access transportation and assist with scheduling transportation.
  • Ensure adherence to transportation service policies and procedures, following protocol for resource distribution.
  • Address transportation requests and calls by providing timely responses to all transportation inquiries.
  • Travel to rural areas (if needed) to provide program services.

Coordination/Provision of Eligible Services

  • Arrange for appropriate levels of educational support and transportation assistance for eligible participants according to program policies and procedures, including use of the most costโ€‘efficient means of conveyance based on individual needs and circumstances.
  • Oversee the distribution of transportation resourcesโ€”bus passes, gas cards, cab rides, etc.
  • Network, refer, and collaborate with internal and external providers to achieve high health outcomes through education, transportation services, and linkage to additional services as needed.
  • Schedule and/or participate in case conferences and reassessment processes.
  • Work with participants and their care teams to achieve healthโ€‘related progress and outcomes, including building independence over time and improving selfโ€‘management skills.
  • Coordinate with all Trillium Health departments to provide transportation as needed.
  • Work with clients to develop understanding and ability to use alternative transportation resources (e.g., Medical Answering Serviceโ€”MAS).
  • Assist with troubleshooting/followโ€‘up on complaints, incident reports, vendor billing reconciliations, and identifying new vendors as needed.

Documentation and Record Keeping

  • Document each medical transportation encounter as a progress note in chronological order in the client chart.
  • Maintain participant records documenting attendance, participation, progress/accomplishments, referrals, and additional needs. Ensure documentation in the electronic health record and required data entry in the AIDS Instituteโ€™s AIRS reporting system.
  • Track the status of transportation services to ensure proper delivery and followโ€‘up.
  • Track and request transportation resources (bus passes, gas cards, etc.) in collaboration with Trillium Healthโ€™s finance department.
  • Maintain spreadsheets and databases for transportation resources, due dates (assessments, reassessments, etc.), and resource utilization.
  • Maintain a process for verifying attendance at allowable healthโ€‘related and HIVโ€‘related medical appointments and document accordingly.
  • Document all educational resources provided or delivered.
  • Assist the Medical Transportation Coordinatorโ€‘2 with required monthly program reports.


Miscellaneous:

  • Requires the ability and commitment to respect and support patients, other employees and visitors with various lived experiences.
  • Requires individual demonstration of commitment to the mission, vision, One Trillium values, behaviors, and modeling them in the organization.
  • Responsible for maintaining confidentiality of all patients, client, employee, protected and proprietary information.
  • Employees are accountable for meeting the performance standards of their departments and must participate as requested in compliance audits and quality improvement plans.
  • Required to participate in continuous improvement of job responsibilities and organizational or work processes to advance the organization or department.
  • Other specific duties as assigned.


Required Skills/Abilities:

  • Knowledge of local and regional transportation systems.
  • Must have a reliable vehicle and meet agency driving policy requirements, including a valid NYS driver's license and required automobile insurance.


Education and Experience:

  • Bachelor's degree in health & human services, Social Work, or a closely related field with 1 year of related experience; OR
  • Associate degree in Health & Human Services, Social Work, or a closely related field with 2 years of related experience; OR
  • High school diploma with 3 years of related experience.
  • Knowledge of local and regional transportation systems.
  • Must have a reliable vehicle and meet agency driving policy requirements, including a valid NYS driver's license and required automobile insurance.


Physical Requirements

While performing the duties of this job, the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms; talk; and hear. Occasionally the employee must stoop, bend, and lift or move up to 25 lbs. Vision requirements include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
