Swift Health Physical Therapy is seeking a dedicated and skilled Full-Time Physical Therapist to join our team in Port Charlotte, Florida. Key Responsibilities
  • Conduct comprehensive evaluations by reviewing patient history and performing specific assessments to devise individualized treatment plans.
  • Effectively communicate evaluation findings and treatment strategies to both patients and their families, encouraging their active participation in goal setting.
  • Implement evidence-based treatment protocols, adapting them to the unique presentations and needs of patients within the evolving healthcare landscape.
  • Collaborate with interdisciplinary healthcare professionals to ensure comprehensive patient care, while monitoring and documenting any necessary adjustments to treatment plans.
  • Oversee all aspects of discharge planning and aftercare follow-up, maintaining thorough and timely documentation of patient progress, including evaluation summaries and discharge evaluations.
  • Uphold efficient time management practices, ensuring compliance with productivity expectations and accurate scheduling.
  • Engage in hospital and community marketing initiatives to enhance visibility and access to our services.
  • Participate actively in departmental goal setting and compliance with accreditation regulations.
  • Commit to continuous professional education by pursuing advanced learning opportunities and serving as a clinical mentor to students.
  • Enhance patient experience through flexible and patient-centric approaches, tackling barriers to accessibility within the community.
Patient Population / Client Focus

Our client focus spans a wide range of patients, including those recovering from orthopedic surgeries, neurological disorders, and sports-related injuries. You will have the opportunity to work with diverse patient demographics in an empathetic and supportive manner, ensuring they receive personalized care that optimally addresses their rehabilitation goals.

Qualifications
  • Current Doctorate degree in Physical Therapy (DPT) is required.
  • Must possess a valid state license to practice as a Physical Therapist in Florida.
  • Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is essential.
  • We value a strong commitment to patient care, communication skills, and a collaborative attitude.
Preferred Experience
  • A minimum of two years of experience as a licensed Physical Therapist is preferred to allow for effective integration into our care team.
  • Experience working in outpatient settings is a plus, providing insights into adaptive practices for community engagement.
  • Prior involvement in research, educational initiatives, or community health programs will be considered advantageous.
