The position is located onsite at our state-of-the-art Rehabilitation Center in South River, New Jersey. Our facility is designed to offer a warm atmosphere conducive to healing and recovery. • Conduct thorough evaluations of patients, reviewing medical history, performing assessments, and documenting treatment outcomes clearly.
• Communicate evaluation results and treatment plans in a compassionate manner to patients and their families, actively involving them in the goal-setting process.
• Implement individualized treatment protocols based on patient assessments and progress while adapting to their evolving healthcare needs.
• Collaborate with other healthcare professionals to ensure a multidisciplinary approach to patient care and timely modifications of treatment plans as necessary.
• Manage all discharge planning processes, ensuring that patients and families are comfortable with their post-rehabilitation plans, while documenting all interactions in compliance with healthcare regulations.
• Maintain effective time management skills to meet productivity expectations while also actively participating in hospital and community marketing initiatives.
• Contribute to departmental goals and ensure compliance with industry regulations, aiming for excellence in patient care.
• Pursue continuing education and professional growth, which includes serving as a mentor for students and new staff.
• Strive to provide an excellent patient experience, offering flexibility and adaptive solutions to meet patient needs.

Our rehabilitation center caters to a diverse population, treating individuals recovering from surgeries, sports injuries, and chronic pain. You will work closely with patients of all ages, assisting them in overcoming physical challenges and achieving their personal health goals through tailored rehabilitation programs and compassionate care.

• Bachelor’s Degree in Physical Therapy or equivalent qualification.
• A valid Physical Therapy license in the State of New Jersey.
• Certification in CPR from the American Heart Association is required.
• Strong interpersonal and communication skills to foster positive relationships with patients and families.

• A minimum of 2 years of clinical experience as a Physical Therapist, preferably in an outpatient setting.
• Experience in using electronic health record (EHR) systems for documentation and patient management.
• Prior involvement in community outreach or marketing efforts will be seen as a great asset.

Swift Health Physical Therapy offers an attractive salary range of USD 100,000 – 120,000 annually, commensurate with experience. In addition to competitive pay, we provide a robust benefits package that includes:
