About the Facility

Ahava Therapy Services is hiring a Full-Time SLP for Cordova Wellness & Rehab Center, a 242-bed skilled nursing facility offering short-term rehab, long-term care, and outpatient therapy services.
Join a supportive, in-house therapy team under outstanding leadership, with a strong focus on collaboration, functional progress, and meaningful clinical impact.

Cordova is a suburban community just outside Memphis, TN, offering a blend of city convenience and family-friendly neighborhoods.
Residents enjoy parks, recreational facilities, shopping, dining, and cultural activities, making Cordova a comfortable and convenient place to work and live.

⚙️ Responsibilities

  • Evaluate and treat residents in accordance with Medicare, Medicaid, and commercial payer guidelines.

  • Conduct comprehensive assessments to determine swallowing, cognitive-communication, and language deficits.

  • Develop and implement individualized treatment plans with functional, achievable goals.

  • Document evaluations, daily treatment, progress notes, and discharge summaries accurately and promptly.

  • Revise treatment plans based on patient performance and progress.

  • Educate residents, families, and caregivers on therapeutic techniques and carryover strategies.

  • Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional abilities.

  • Participate in care conferences, facility meetings, and quality improvement activities.

  • Maintain up-to-date knowledge of community resources and assist with discharge planning as needed.

  • Stay clinically current through continuing education and contribute to in-service education.

  • Adhere to professional, ethical, regulatory, and facility standards.

  • Maintain expected productivity levels and demonstrate strong teamwork, communication, and clinical judgment.

✔️ Qualifications

  • Master’s Degree in Speech-Language Pathology

  • Will accept CFYs

  • Active SLP license liscense in the state of practice

  • Current BLS/CPR certification

  • Strong observational, verbal, and written communication skills
