Key Responsibilities
  • Conduct patient assessments to identify social service needs.
  • Develop and implement personalized care plans in collaboration with an interdisciplinary team.
  • Provide emotional support and counseling to patients and families.
  • Advocate for patient needs within the facility and coordinate with external resources.
  • Maintain accurate and confidential patient records.
  • Oversee discharge planning and ensure continuity of care post-discharge.

Qualifications
  • Master’s degree in social work (MSW) required
  • Social Work License valid in the state of AL
  • Minimum 1 year of experience in Social Work or a related field.
  • Experience in a long-term care or healthcare setting, particularly in social services.
  • Strong understanding of case management and care coordination.
  • Exceptional communication and interpersonal skills.
