Vitality Care Clinic is seeking a dedicated and dynamic Full-Time Physical Therapist (PT) to become an integral part of our outpatient team in West Hartford, Connecticut. Work Environment

This role is conducted onsite at our state-of-the-art outpatient facility, designed to foster collaboration and community among healthcare providers. Working Monday through Friday from 8 AM to 3 PM, our physical therapists engage in a vibrant and supportive atmosphere where teamwork is not only encouraged but essential in delivering exceptional patient care.

Key Responsibilities
  • Conduct thorough evaluations and assessments of patients, particularly children, to design appropriate physical therapy interventions.
  • Develop and implement personalized treatment plans that focus on specific patient goals and objectives, ensuring that each plan is regularly reviewed and adapted based on therapeutic progress.
  • Perform regular reassessments of patients' clinical signs and symptoms to evaluate the effectiveness of therapy and adjust care plans accordingly.
  • Document all clinical interactions and patient progress using the company's electronic record system, including SOAP notes, in a timely and organized fashion.
  • Educate patients and their families on the rehabilitation process, setting realistic expectations while addressing any concerns or challenges faced during recovery.
  • Meet established utilization goals and contribute to other business objectives aimed at enhancing clinic efficiency and patient satisfaction.
  • Collaborate with multidisciplinary peers within the clinic to provide continuity of care for patients receiving multiple therapy services.
  • Actively promote the clinic's culture and core values while representing Vitality Care Clinic as an ambassador in the community.

Qualifications
  • A Master's degree or Doctor of Physical Therapy (DPT) from an accredited physical therapy program is required.
