PRN RATES ARE BASED ON EXPERIENCE:

*RN- $125-$150 /per OASIS - $60-$75 /per visit

*LVN- $40-$60 /per visit

*CNA- $25-$45 /per visit

PT/OT- $60-$75 /per visit

PTA's / COTA's- $50-$60/ per visit

FULL TIME RATES ARE BASED ON EXPERIENCE and COVERAGE AREA

RN: $78,000 - $90,000.00 annual salary

LVN: $60,000 - $75,000 annual salary

IMMEDIATE OPENINGS for FULL-TIME:

RN or LVN in Northeast Houston

RN in West Houston

RN or LVN in South / Southwest Houston

**Position pays on a bi-weekly basis as a W-2 employee**

Qualifications:

• Previous Home Health Care experience is required

• Working Experience of Kinnser EMR software

•Current & unremarkable RN/LVN License - CNA cert

•Current BLS/CPR certification

• Comfortable in a role working alone without any assistance

•Must be willing and able to travel to multiple patient's homes daily and provide quality care and follow up while being efficient

We are looking for an empathetic and passionate Home Health Nurses, (RN and LVN) Certified Nurse Aides to assist a growing home healthcare company.

Our patients are homebound and mostly elderly and ill. It is necessary to provide each patient with the time and attention necessary to meet his or her needs.

Job Specific Accountabilities The statements below describe the general nature and level of work being performed in this role. They are not intended to be a complete list of all duties, and additional responsibilities may be delegated as required by the Physician or supervisor.

Notes: The position will require travel within the city of Houston/Angleton as well as their contingent counties. Must work with other healthcare professionals to create treatment plans. You will be responsible for but not limited to monitoring patient's vital signs, wound care, foley care, monitor changes in behavior and progress, as well as monitoring and administers medication. Communication with a referring physician and participates in professional development activities will also be required. Skill level required will vary depending on patient needs and will include training on G-tube care, trach care, wound care and foley care as well as ADL's, and other skills. Seeking skilled nurses with the comfortability of working independently and familiarity with OASIS, for RNs.

RN's - Responsible for the admission, planning, intervention, and ongoing evaluation of patients in home care. Will work cohesively with LVN and/or CNA in designated coverage areas.
