Home Care Field Supervisor - Registered Nurse (RN)

We are seeking a compassionate, dedicated, and highly skilled Registered Nurse (RN) to join our home care team as a Home Care Field Supervisor. This role is essential in ensuring our clients receive exceptional, safe, and consistent care in the comfort of their homes. As a Field Supervisor, you will provide clinical oversight, support caregivers in the field, and ensure all services meet state regulations and agency standards.

Key Responsibilities
  • Conduct in-home supervisory visits with clients to assess care quality, safety, and client satisfaction.

  • Develop, update, and implement individualized care plans according to state requirements and agency guidelines.
  • Train, mentor, and support field staff-including HHAs, CNAs, and other caregivers-to ensure competency and compliance.
  • Provide clinical guidance and act as a resource for caregivers regarding client needs, care techniques, and best practices.
  • Monitor documentation for accuracy, completeness, and compliance with state regulations.
  • Maintain timely communication with office staff, clients, family members, and care teams.
  • Participate in on-call rotation as needed to support the agency and ensure continuity of care.
  • Identify concerns in the field and take appropriate corrective actions to maintain high standards of service.
  • Ensure all care is delivered in accordance with company policies, licensing standards, and ethical practices.
Qualifications
  • Must hold an active, unrestricted Registered Nurse (RN) license.
  • Minimum 1 year of nursing experience required (home care experience preferred).
  • Strong understanding of home care regulations, care plans, and assessment procedures.
