We are Bend Transitional Care, a 60-bed skilled nursing and rehabilitation facility in Bend, Oregon that provides transitional, post-hospital care for people who need extra medical support before returning home or moving to longer-term care. We're dedicated to providing compassionate, high-quality skilled nursing and rehabilitation care that supports our residents in living their best lives. We're looking for a dependable and caring Full Time Registered Nurse (RN) to join our nursing team.


Schedule Requirement:

This position works three 12-hour shifts per week from 6:00 PM - 6:30 AM. Applicants must be available to work this set schedule.ย

Position Summary

The primary role of the Registered Nurse is to supervise staff, monitor patients, implement treatments, develop patient care plans for long-term needs, administer medications, follow-up with families and provide the highest quality of care to all residents/patients. Provides clinical care in compliance with current federal and state guidelines, and company regulations.

Note: All employees of Cascadia Healthcare are required to submit and be cleared to work in the facility per eachย 

state's specific background check requirements prior to contact with patients/residents.


Essential Functions

  • ย Provides high-quality care to residents/patients.ย 
  • Demonstrates sound clinical judgement.
  • Documents care, assessments, and resident status accurately and timely in compliance with facility policies and all regulatory requirements.ย 
  • Communicates changes in resident condition with physicians and care team members.ย 
  • Completes all required nursing documentation, including assessments, orders, progress notes, and flowsheets.ย 
  • Manages medications safely by administering, documenting, monitoring resident responses, reporting adverse reactions, maintaining narcotic records, and ensuring medication availability and order compliance.ย 
  • Responds to emergencies, initiates appropriate interventions, and performs CPR when needed.ย 
  • Maintains punctual and regular attendance for assigned shifts.

Other Functions

  • Ensures compliance with company's clinical policies and procedures.
  • Follows the guidelines established from the Nurse Practice Act, company standards, policies and procedures and sound nursing judgment.
  • Performs other related duties, as assigned.


Knowledge/Skills/Abilities

  • Must maintain confidentiality and protect resident rights, regarding all resident/personnel information.
  • Strong knowledge of HIPAA laws and related regulations.
  • Must possess strong communication, customer service, teamwork and organizational skills.
  • Positive, enthusiastic attitude.

Requirements

Education

Graduate of an accredited RN School of Nursing. BSN preferred.


Licenses/Certification ย 

Current RN Licensure in the State of Oregon

Valid CPR Certification


Experience

Six months experience in a long-term care environment preferred.
