Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and experienced Field RN Case Manager to provide skilled nursing care and case management services to patients in their homes throughout the Tyler area. The RN Case Manager will assess patient needs, develop individualized care plans, coordinate interdisciplinary services, and ensure high-quality patient outcomes while maintaining compliance with Medicare, state, and agency regulations.
Responsibilities
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, and Discharge assessments.
  • Develop, implement, and update individualized patient care plans based on physician orders and patient needs.
  • Provide skilled nursing care, including medication administration, wound care, IV therapy, patient assessments, and disease management.
  • Coordinate patient care with physicians, therapists, home health aides, and other members of the interdisciplinary care team.
  • Monitor patient progress, identify changes in condition, and communicate updates promptly to physicians and the clinical team.
  • Educate patients and caregivers on medications, disease management, safety, infection prevention, and self-care.
  • Complete accurate and timely OASIS documentation while ensuring compliance with Medicare, state, and agency standards.
  • Participate in interdisciplinary case conferences and maintain productivity while delivering compassionate, patient-centered care.
Qualifications
  • Current, unrestricted Registered Nurse (RN) license in the State of Texas.
  • Minimum of 1 year of nursing experience required; home health experience preferred.
  • Previous case management experience in home health is highly preferred.
  • Strong knowledge of Medicare home health regulations and OASIS documentation.
  • Excellent assessment, clinical decision-making, and critical-thinking skills.
  • Strong communication, organizational, and time-management skills.
  • Proficiency with electronic medical records (EMR).
  • Current BLS/CPR certification.
  • Valid Texas driver's license, reliable transportation, and current auto insurance.
