• Conduct thorough mental health assessments for patients in the hospice setting, evaluating their psychological and emotional needs along with their physical health conditions.
• Develop personalized care plans in collaboration with patients and their families, focusing on emotional support, grief counseling, and coping strategies tailored to the terminal nature of their conditions.
• Utilize evidence-based therapeutic techniques, such as Cognitive Behavioral Therapy (CBT) and supportive counseling, to address issues related to terminal illness, grief, and bereavement.
• Facilitate individual and family therapy sessions via telehealth, helping families communicate effectively about difficult topics and enhancing emotional resilience.
• Coordinate closely with other healthcare team members including physicians, nurses, and spiritual care providers to ensure a holistic approach to each patient’s care.
• Offer crisis intervention as needed, employing de-escalation techniques to provide immediate support during difficult emotional moments.
• Educate patients and families about mental health resources and strategies for coping with grief and loss, empowering them to navigate their emotional journey.
• Maintain meticulous documentation of patient interactions, treatment plans, and progress notes in compliance with legal, ethical standards, and organizational policies.
• Engage in regular team meetings to share insights, coordinate care, and participate in continuous professional development to enhance best practices.

Your primary focus will be on patients receiving hospice care—individuals facing terminal diagnoses, alongside their families who may be experiencing profound emotional distress. You will be instrumental in providing not only therapeutic support but also fostering a comforting presence that considers the unique needs of each individual’s journey towards the end of life.

• A Master’s degree in Social Work from an accredited program.
• Current and valid Licensed Independent Clinical Social Worker (LICSW) licensure in Washington state.
• A minimum of two years of post-licensure experience, ideally within hospice, palliative, or mental health settings.
• In-depth knowledge of mental health disorders, grief counseling practices, and trauma-informed care.
• Demonstrated effectiveness in conducting assessments and developing treatment plans that meet the needs of diverse populations.

• Experience with teletherapy platforms and electronic health record systems.
• Previous engagement in a hospice or end-of-life care context.
• Proficiency in facilitating group therapy sessions and family interventions.
