• Conduct comprehensive clinical assessments to evaluate patients’ needs and create personalized treatment plans based on solution-focused methods.

• Implement evidence-based interventions tailored to meet individual patient needs, focusing on enhancing their well-being and coping strategies.

• Maintain accurate clinical documentation and records, ensuring they align with regulatory standards and organizational policies.

• Collaborate closely with a multidisciplinary team, participating in regular case consultations, team meetings, and peer reviews to foster integrated care approaches.

• Provide education and support to patients and families, empowering them with the skills and knowledge necessary to navigate mental health challenges.

In this role, you will work primarily with diverse patients facing various psychosocial challenges associated with serious illnesses. Providing emotional support to patients and their families during the hospice phase requires compassion, patience, and an understanding of end-of-life issues. You will play a pivotal role in helping clients achieve their treatment goals and improve their overall quality of life through targeted interventions.

• A Master’s degree in Social Work from an accredited institution.

• A valid and active LICSW license in the state of Massachusetts.

• At least three years of post-master’s clinical experience, with a focus on psychotherapy and supportive care.

• Strong proficiency in solution-based therapeutic interventions, with a commitment to patient-centered practices.

• Exceptional communication and interpersonal skills, enabling effective collaboration with colleagues, patients, and families.

• Ability to thrive in a multidisciplinary team environment, fostering cooperation and shared goals.

• Experience in a hospice or palliative care setting, demonstrating knowledge of end-of-life issues.

• Familiarity with electronic health record systems to streamline documentation and care processes.
