• Conduct thorough clinical assessments of patients to identify mental health needs and formulate individualized treatment plans that address those needs.
• Implement evidence-based therapeutic interventions, including solution-focused techniques, cognitive behavioral therapy, and motivational interviewing, tailored to the unique challenges of each patient.
• Maintain detailed and timely clinical documentation, ensuring that all records are current and compliant with regulatory standards.
• Collaborate with a multidisciplinary team, including nurses, physicians, and other healthcare providers, to develop comprehensive treatment strategies and ensure seamless care coordination.
• Provide psychoeducation and support to patients and their families, equipping them with coping strategies and resources to navigate mental health challenges.

As a Licensed Clinical Social Worker at Gotham Enterprises, you will work primarily with patients receiving hospice care, often facing terminal diagnoses. • A Master's degree in Social Work from an accredited institution.
