This position allows for remote work while providing support to individuals in a detox center. • Conduct thorough assessments of patients to identify psychosocial issues and develop tailored treatment plans.
• Provide individual and group counseling sessions, utilizing evidence-based practices to facilitate recovery.
• Assist patients in navigating the complexities of accessing health benefits, including completion of entitlement forms and navigating insurance options.
• Advocate for patients by collaborating with community agencies to secure necessary support and resources.
• Maintain meticulous patient records and documentation in compliance with healthcare regulations and standards, ensuring accurate reporting and follow-up.
• Coordinate patient transportation and other logistical support to enhance accessibility to care.
• Engage in case conferences and team meetings, contributing insights and strategies to improve patient outcomes.
• Facilitate patient education programs, particularly focusing on prenatal and postpartum health, as well as HIV counseling.
• Conduct regular follow-ups with patients to monitor progress and address concerns such as missed appointments.
• Participate in Quality Improvement initiatives to enhance service delivery and clinical outcomes.

In this role, you will primarily work with adults experiencing substance use disorders as well as co-occurring mental health conditions. These clients may come from diverse backgrounds and have varying levels of readiness for change, requiring a compassionate and individualized approach to counseling.

• A minimum of a Bachelor’s Degree in Social Work or a related field is required.
• Licensure as a Clinical Social Worker (LCSW) is preferred, showcasing a commitment to high standards of practice.
• Strong interpersonal, written, and verbal communication skills are imperative to connect effectively with patients and colleagues.
• Proficiency in Microsoft Office Suite, including MS Word and Excel, is essential for documentation and reporting.

• Two to five years of relevant experience as a Social Worker, bringing a wealth of knowledge in case management and counseling.
• Experience in a healthcare setting, particularly with detox or mental health services, is highly desirable.
• Familiarity with healthcare regulations and documentation practices is beneficial.

This position offers a competitive salary ranging from USD 115,000 to 130,000 annually, commensurate with experience.
