Job description

Job Title: Virtual Care Coordinator
Location: New York, NY
Job Type: Full-Time Contract
Contract Duration: 3 Months (Possible Extension)
Pay Rate: $26/hour (W2)
Schedule
  • Full-Time - 40 Hours per Week
  • Shift: 11:00 PM - 7:00 AM
  • Every Other Weekend Required
Position Overview
We are seeking an experienced Virtual Care Coordinator to support virtual patient care operations through telehealth and remote patient monitoring programs. This role serves as a key member of the virtual healthcare team, ensuring patient safety, coordinating telehealth services, providing technical support, and assisting with quality improvement initiatives.
The ideal candidate will have a strong background in direct patient care, telehealth operations, remote monitoring technologies, and healthcare workflow optimization.
Key Responsibilities
Telehealth Program Coordination
  • Coordinate telehealth and remote patient monitoring services.
  • Monitor consult orders, virtual visits, and follow-up appointments.
  • Ensure telehealth sessions run smoothly and without technical interruptions.
  • Support patient enrollment and transitions into virtual care programs.
  • Assist with telehealth workflow development and implementation.
Patient Monitoring & Safety
  • Provide continuous virtual observation of assigned patients.
  • Monitor patient safety indicators and identify potential risks.
  • Recognize changes in patient condition and escalate concerns promptly.
  • Document observations, interventions, and safety events.
  • Support appropriate discharge from remote monitoring programs when indicated.
Quality Improvement & Program Oversight
  • Conduct quality assurance reviews and program evaluations.
  • Analyze utilization data, performance metrics, and patient outcomes.
  • Identify opportunities for process improvement.
  • Prepare reports for leadership and participate in performance review meetings.
  • Assist with workflow optimization and corrective action initiatives.
Technical & Clinical Support
  • Provide support for telehealth platforms and remote monitoring equipment.
  • Troubleshoot technical issues affecting patient care delivery.
  • Collaborate with technology teams and vendors to resolve system concerns.
  • Assist with onboarding and training staff on telehealth workflows and systems.
Communication & Collaboration
  • Serve as a liaison between clinicians, physicians, technology teams, and vendors.
  • Communicate effectively with patients, families, and interdisciplinary teams.
  • Participate in team meetings and organizational initiatives.
  • Support communication and interpretation needs through approved channels.
Compliance & Documentation
  • Maintain HIPAA compliance and patient confidentiality.
  • Accurately document monitoring activities and interventions.
  • Escalate safety concerns and equipment issues appropriately.
  • Support a safe and compliant virtual care environment.
Required Qualifications
Education
  • Associate Degree required
  • Bachelor's Degree in a healthcare-related field preferred
Experience
  • Minimum 3 years of direct patient care experience
  • Minimum 2 years of telehealth experience required
  • Experience with remote patient monitoring technologies
  • Experience with EMR/EHR systems (Epic preferred)
  • Experience developing telehealth workflows, policies, and protocols
  • Experience with quality improvement initiatives and performance metrics
  • Experience collaborating with multidisciplinary healthcare teams
Knowledge & Skills
  • Strong understanding of telehealth operations and regulatory requirements
  • Ability to analyze data and identify improvement opportunities
  • Knowledge of quality assurance and quality improvement methodologies
  • Strong clinical judgment and patient safety awareness
  • Excellent problem-solving and critical-thinking abilities
  • Strong written and verbal communication skills
  • Ability to work across multiple technology platforms simultaneously
  • Understanding of cultural, language, and accessibility considerations in healthcare delivery
Language Requirement
  • English (Read, Write, and Speak)
Preferred Qualifications
  • Associate or Bachelor's Degree in a healthcare-related field
  • Experience supporting large-scale telehealth programs
  • Experience educating patients and providers on telehealth services
  • Experience measuring and reporting telehealth program outcomes and success metrics
