Key Responsibilities
  • Conduct ongoing assessments and evaluations to accurately identify and address the needs of clients.

  • Create and provide a tentative schedule for client appointments, home visits, and necessary observations to supervisors.
  • Coordinate all aspects of home visits, which may include conducting observations, reviewing client charts, and interviewing staff for comprehensive data analysis.
  • Implement and monitor the effectiveness of patients' program plans, ensuring that therapeutic interventions are executed appropriately.
  • Engage actively in interdisciplinary team meetings to discuss client progress, share insights, and improve service delivery.
  • Document and complete monthly and quarterly progress notes to track client improvement and program effectiveness.
  • Organize training sessions for staff and team members to ensure competence in executing the program plan and proper documentation processes.
