• Conduct thorough assessments and ongoing evaluations tailored to meet each patient's unique mental health needs.
• Maintain proactive communication with supervisors, interdisciplinary teams, and the patients' families to discuss progress and adjust treatment plans accordingly.
• Develop schedules for client appointments, home visits, and observations, providing a structured approach to monitoring patient progress.
• Coordinate essential home visits, including site observations and data analysis, to ensure a comprehensive understanding of patients' challenges in their familiar environments.
• Ensure that all program plans are effectively implemented, tracking their efficacy through regular follow-ups and documentation.
• Engage in interdisciplinary team meetings, sharing insights and strategies that enhance collaborative care.
• Document monthly and quarterly progress notes to track client development and inform treatment adjustments.
• Organize and lead training sessions for staff, ensuring proficiency in program plan implementation and adherence to documentation standards.
• Provide ongoing support through follow-up interviews, home visits, and observations to foster sustained improvements in client well-being.
• Act as a liaison between the organization and the broader mental health community, advocating for resources and support that enhance program goals.

In this role, you will work with a diverse population, including children, adolescents, and adults who are navigating various mental health challenges. Whether addressing anxiety, depression, trauma recovery, or substance misuse, your dedication to personalized care will be critical in creating a supportive environment that encourages healing and growth.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
• Active and unrestricted licensure as a Professional Counselor (LPC) in Colorado.
• A minimum of 2 years of experience in a clinical counseling setting.
• Strong understanding of ethical and legal standards within the field of mental health.
• Outstanding interpersonal and communication skills; a proven ability to build rapport with clients from diverse backgrounds.
• Demonstrated capability to work collaboratively within a healthcare team.
• Proficiency in utilizing EMR (Electronic Medical Records) software and related technologies.

• Specialization in areas such as family therapy, couples therapy, trauma-informed practices, or substance use interventions is highly desired.
• Previous experience working with multidisciplinary teams and incorporating varied treatment modalities into practice.
