Key Responsibilities
  • Conduct thorough psychosocial assessments to identify clients' mental health needs, strengths, and resources, ensuring a comprehensive understanding of their unique situations.
  • Deliver individual, family, and group therapy using evidence-based treatment modalities tailored to meet the emotional and psychological needs of our hospice clientele.
  • Implement therapeutic techniques such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care, with an emphasis on building resilience and coping mechanisms.
  • Create and monitor individualized treatment plans, outlining specific goals and interventions, actively involving clients and families in their healthcare journey.
  • Engage in interdisciplinary collaboration with psychiatrists, psychologists, and other professionals to coordinate comprehensive care for clients at all levels.
  • Provide crisis intervention and risk assessment as needed to ensure client safety and well-being.

This role primarily focuses on individuals receiving hospice care who may be experiencing concurrent mental health issues. Understanding and addressing the complex emotional needs associated with end-of-life care is vital to enhancing both client and family experiences.

Qualifications
  • A Master's degree in Social Work from a CSWE-accredited program is required.
  • An active state license as a Licensed Clinical Social Worker (LCSW) is essential.
  • A minimum of two years of experience in an outpatient mental health or hospice setting is preferred.
  • A robust understanding of evidence-based therapeutic modalities and treatment approaches to cater effectively to diverse client needs.
