• Conduct thorough assessments and create tailored treatment plans reflecting patient needs and preferences.
• Engage in crisis intervention planning and develop behavioral guidelines, risk assessments, and medication treatment strategies as necessary.
• Document all clinical encounters efficiently, maintaining compliance with regulatory standards and organizational protocols.
• Implement evidence-based screening tools in collaboration with the clinical team to enhance patient assessments.
• Participate actively in team discussions to develop and implement individualized positive behavior support plans.
• Act as a liaison with relevant service systems, state agencies, and community partners to facilitate patient access to required resources.
• Communicate effectively with internal teams and external stakeholders to ensure continuity of care and promote the mission of the center.
• Build and sustain relationships with local, state, and federal agencies to identify and secure potential partnerships and funding opportunities.
• Engage in ongoing professional development activities, including attending relevant workshops, conferences, and statewide meetings.
• Contribute positively to the agency’s mission and core values, promoting a culture of collaboration and support.
• Perform additional duties as assigned by the Clinic Director, always aligning efforts with the organization’s goals.
