Description

We are currently seeking qualified candidates for a full time Long Term Care Registered Nurse (RN) position in the Houston area.


Job Description:
 
The hospice LTC RN is an integral part of the interdisciplinary care team. Responsible for all duties as outlined by the job description as well as the following responsibilities as listed below. Understanding that care must be delivered according to accepted standards of practice, guided by the mission of the agency and the patient's and family's needs and concerns. The LTC RN blends all services of the agency into an integrated program of support. The LTC RN also guides their immediate healthcare team in adhering to these standards by monitoring the professionalism and quality of care given by the healthcare team to which they are assigned.
 

Qualifications:
  • Licensed Registered Nurse (RN) in the State of Texas - required
  • Must have a BSN or equivalent RN experience
  • Must have training in Long Term Care and the disease processes leading to death, control of pain and other symptoms
  • Strong EMR/documentation skills with compliance awareness - required
  • Experience serving patients in Long Term Care settings (NF, ALF, Memory Care, personal care homes) - preferred
  • Highly Organized and Detail Oriented
  • Effective written and verbal communication
  • The ability to travel using reliable transportation
  • Clean background and drug screen
