Location : Pocopson Home - West Chester, PA
Job Type: Full Time, Non Exempt

Closing Date: Continuous
Weekly Hours: 38.75
Shift: 3 pm - 11 pm
Summary
The Licensed Practical Nurse performs and provides prescribed medical treatment and personal care to residents in a skilled nursing setting. Assists the Unit Manager and Supervisor with ensuring continuity of care between the shifts. The LPN is also responsible for supervising Certified Nursing Assistants and other supportive staff members working on the nursing unit. Maintains role as resident advocate with a focus on the facility's mission.
Essential Duties
  • Adequately assess residents for pain and administer medications as ordered.
  • Adequately provides ongoing assessment of the resident after the RN has completed the initial assessment
  • Perform all aspects of resident care in an environment that optimizes resident safety.
  • Collect samples, such as urine and sputum. Also completes glucose testing on residents as ordered.
  • Completes written documentation and assessments in a timely, clear, and concise manner.
  • Complete treatments as ordered.
  • Assemble and use equipment such as catheters, tracheotomy tubes, and oxygen suppliers.
  • Note and carry out physician orders in a timely manner.
  • Administer medications according standards set forth in the Nurse Practice Act including but not limited to oral, intramuscular, or subcutaneous.
  • Monitor for adverse effects of medication and make physician aware.
  • Communicate residents' status and/or needs to the supervisor or charge nurse, physician, consultants and Nurses' Assistants.
  • Supervise activities by directing, communicating and monitoring Certified Nurse Assistants (CNA's).
  • Perform other duties, tasks and special projects, as required

Qualifications/Preferred Skills, Knowledge & Experience
  • Graduate from an approved LPN program
  • An unrestricted LPN license
  • Current Certification in CPR.
  • Six months to one year of job-related experience and/or training.
  • Mandated in-services to meet state regulations.
  • Educational and clinical experience required by State Nursing Board.
  • Excellent interpersonal skills.
  • Strong organizational skills.
  • Ability to work independently and proceed with objectives.
  • Ability to maintain confidential information and handle confidential matters.
  • Strong professional ethics.
Preferred Skills:
  • Attend in-services and seminars related to geriatric population and wound care.
  • Ability to complete continuing education and remain updated on the health care field via classes, seminars, magazines, journals, literature and videos.
  • Working knowledge of Pocopson Home policies and procedures.
  • Strong time management skills.
  • Working knowledge of Nursing Board regulations.
  • Flexibility and adaptability to change.
  • Ability to work effectively with people from diverse backgrounds.
  • Treats others with respect and consideration regardless of their status or position.
